ReliOn Community – FREE patient education resources
Overview of ReliOn Community
ReliOn Community is a free text-based education program in English and Spanish for patients using a ReliOn Premier Blood Glucose Monitor. It sends helpful diabetes tips, recipes and motivations right to your phone, making it easier to live a healthy life with diabetes.
- Patients using a ReliOn Premier meter can sign up for the program at relionbgm.com/community
- Providers can try a condensed version of the program to get a feel for format and content at relionbgm.com/lp7
ReliOn Community counter easel
The ReliOn Community counter easel (8.5×11) helps explain the program to patients and can be set in your practice. Tear pads can be attached to the easel.
- Easels come with (x1) stack of tear pads with a sticky back that can be applied to the easel
- Order additional tear pads to refill the easel separately
- Limit (x1) easel per quarter
ReliOn Community tear pads
The ReliOn Community tear pads come in a stack of thirty and give an overview of the program with sign-up information. These are double-sided in English and Spanish.
